Covid-19 Pre-Appointment Checklist
Before you confirm your appointment please read (and check) the following. If you cannot confirm any of the following, please call 561-833-7411 to reschedule your appointment.
You confirm that you do not have any of the following symptoms: (i) fever; (ii) chills; (iii) loss of smell or taste;(iv) shortness of breath; (v) dry cough; (vi) muscle pain; (vii) runny or congested nose; or (viii) sore throat.
You confirm that you have not been in contact with a person who has been diagnosed with COVID-19 or any person who believes that he or she may have been exposed to the SARS-CoV-2 virus.
You confirm that you have not been diagnosed as having contracted the SARS-CoV-2 virus (or diagnosed with COVID-19) nor are you currently awaiting test results for the SARS-CoV-2 virus.
You confirm that you have not traveled internationally within the past thirty (30) days nor have you traveled domestically by commercial airline, bus or train within the past thirty (30) days.
You agree to abide by all health-related laws, guidelines, rules and ordinances issued by any federal, state or local health authorities at all times.
